HLA-DR Expression on Monocytes and Sepsis Index Are Useful in Predicting Sepsis

چکیده
The reduction of mortality in patients with sepsis depends on the early identification and treatment at-risk patients. aim was to evaluate HLA-DR expression surface monocytes (MHLA-DR ratio), index (CD64 neutrophils/MHLA-DR C-reactive protein (CRP) development sepsis. We prospectively enrolled 77 critically ill patients, 59 stroke 18 traumatic brain injuries. biomarkers were tested at baseline 3, 6, 9, 12, 15 days later. Most (71%) developed (4.2 ± 1.3 after admission). On day those subsequently developing had lower levels MHLA-DR+ (81.7 16.2% vs. 88.5 12.1%, p < 0.05) higher indexes (0.19 0.19 0.08 0.08, 0.01) than not MHLA-DR ratio slowly recovered before while remained raised septic up 9 (p 0.05). To predict sepsis, optimal cut-offs CRP > 106.90 mg/mL (74.19% sensitivity, 69.49 specificity) rate 72.80% (45.31% 89.47% specificity). periodic monitoring together may help identify ICU increased risk

HLA-DR expression, cytokines and bioactive lipids in sepsis
Sepsis accounts for more than 200,000 deaths annually in the USA alone. Both inflammatory and anti-inflammatory responses occur simultaneously in sepsis, the early phase dominated by the hyperinflammatory response and the late phase by immunosuppression. This late immunosuppression phase leads to loss of the delayed type hypersensitivity response, failure to clear the primary infection and deve...
متن کاملHLA-DR expression on monocytes is decreased in polytraumatized patients.
BACKGROUND Sepsis, systemic inflammatory response syndrome (SIRS) and multiple organ dysfunction syndrome (MODS) remain the most frequent causes of complications and death in severely injured patients. A main reason for the development of these syndromes is a post-traumatic dysregulation of the immune system. Several studies in intensive care unit (ICU) patients could detect a pivotal role of H...
متن کاملDynamic monitoring of monocyte HLA-DR expression for the diagnosis, prognosis, and prediction of sepsis.
Sepsis, an under-recognized health problem, is a major cause of death. More than 750,000 individuals develop sepsis annually, of whom 215,000 die of the disease. Clinical and experimental evidence indicates that patients with sepsis present with rapid impairment of immune function; biomarkers are therefore needed to enable early detection of this condition. Reduced monocyte human leukocyte ant...
متن کاملserum inflammatory markers in the elderly: are they useful in differentiating sepsis from sirs?
differentiating sepsis from other noninfectious causes of systemic inflammation is often difficult in the elderly. the aim of this study was to evaluate the ability of c-reactive protein (crp), erythrocyte sedimentation rate (esr), procalcitonin (pct), and interleukin-6 (il-6) to identify elderly patients with sepsis. in this single center prospective observational study, we included all consec...
متن کاملPredicting Funisitis and Neonatal Sepsis
Objectives: To evaluate the usefulness of serum C-reactive protein (CRP) in women with preterm prelabour rupture of membranes (PPROM) in the prediction of funisitis and early-onset neonatal sepsis (EONS), to determine a CRP cutoff value for their prediction, and to identify other significant risk factors associated with funisitis and EONS. Methods: This was a retrospective study conducted in a ...
